Interfaith Community Services (ICS) is a 30-year-old non-profit 501(c)(3) organization that provides services for seniors, adults with disabilities, and individuals facing financial crises in the greater Tucson community. Essential safety-net services include emergency financial assistance for rent, utilities and other essential items; emergency food boxes and supplies for low-income families and individuals; a resource center for employment assistance, financial literacy, and job coaching; and transportation, caregiving services, and health advocacy for seniors and disabled individuals. ICS is supported by 700 volunteers, 91 faith communities, and many community partners. DESCRIPTION The part-time Program Coordinator will be responsible to help implement and support daily operations of a newly funded initiative, Care Partners, a program of the ICS Faith and Community Health Network working under the direction of the ICS Health & Senior Services Director. The Faith and Community Health Network (FCHN) brings together faith communities, healthcare system and Interfaith Community Services (ICS) to deliver help and hope to people in need. With the focus on mind, body, spirit and community, the project aims to increase the capacity and strengthen the whole health programs of faith communities and expand their capacity to reach outside their walls to assist neighbors in need of nonmedical support. DUTIES AND RESPONSIBILITIES:  Assist in the daily operations of ICS Care Partners to respond to partner calls for support  Serve as a designated ICS point of contact for hospital staff and faith community liaisons  Promote effective communication and interactions between hospitals, faith communities and internal ICS departments  Assist in motivation, management, and support of volunteers in the ICS Care Partners program  Manage updates and changes to program forms and policy & procedures  Enter data from source documents into ETO software database, files and forms  Assist with grant reports  Maintain positive working relationships and regular communication with all program partners  Maintain detailed records of program activities and help assure Care Partners program achieves proposed outputs and outcomes. ADDITIONAL DUTIES:  Attend monthly staff meeting, meetings for Health & Senior Services team and other meetings  Participate in ICS volunteer recruitment and recognition, fund raising, outreach, health fairs and other agency projects as needed. QUALIFICATIONS: Background in nursing, social services or case management. Experience working with volunteers. Comfortable in both faith community and healthcare settings. Must be a self-starter and capable of helping to launch a highly visible new whole health initiative with ICS team. Excellent written and verbal communication skills.
